Request Graduate Program Contact and Content Changes

In the field below, please list the complete program names as they appear on the Graduate School webpages, found at Graduate Programs, for any pages requiring updates (include master’s, doctoral , and certificate pages).

Please describe the updates to program contacts you are requesting (e.g., replacements of existing contacts, or updates to existing contacts, temporary changes due to contact taking leave).

Department head/director and academic contact roles are required for each program. One contact for each of three contact roles may be published per program page on the Graduate School website:

• department head or director
• academic contact
• admissions contact

Department head phone numbers and email address are not published. List the phone number and official university @colostate email address for each academic and admissions contact in the field below. The official @colostate email address of an individual may be published or substituted with a distribution list address. (Use of a distribution list is encouraged so a program can manage the distribution of communications within their program.)
